Garden Village, in partnership with the Mid North Coast Local Health District (MNCLHD), has launched the Aged Care Outreach Support (ACOS) Pilot Program through MNC Virtual Care.
“This is a fantastic step forward in enhancing the way we deliver clinical care. It is available when residents become acutely unwell and their GP is unavailable or to avoid a non urgent hospital visit,” said Tammie Gough Aged Care General Manager.
The pilot program started on Monday, 4 August 2025. Aged Care Outreach Support offers virtual support for nursing and medical assessments, a mobile nurse flying squad, and assistance with prescribing medications for acute cases including ordering pathology and medical imaging.
“This pilot program is a wonderful opportunity for residents to remain in their home whilst receiving outreach services from the hospital and preventing transfer and possible admission to the hospital,” said Craig Wearne CEO Garden Village.
Garden Village Nurses received training on how the model of care can support clinical practice.
