Step One

Talk to Garden Village friendly Community Team

Step Three

Assessment at your home 

You may be referred for a face-to-face assessment with the Aged Care Assessment Team (ACAT). An assessor will visit you at your own home or contact you by phone to understand your support needs. A family member, friend or carer can be with you during your assessment.

Step Four

Your Package is Approved  

You will receive a confirmation letter and once you have received your My Aged Care approval, your name will be placed on the national Home Care Package waitlist.

Step Five

Start your Garden Village Home Care Services

Once you receive your Home Care Package call us. Together we will develop your individual Care Plan tailored to your goals and needs, and match you with compatible Support Workers

 

Your Home Care Package

When you choose Garden Village Port Macquarie as your Home Care Provider, we will contact you to organise a home visit. Our goal is to get to know you and give you the opportunity to get to know us. If you wish, you can include loved ones in the process of planning your Home Care Package. 

Your Home Care Package is based on your needs and your health. Together we will identify your needs and goals, and create a personal Home Care Plan that’s right for you.

This plan will form the basis of your Home Care Agreement with us. Your Home Care Agreement will include:

  • What services you will receive
  • When you will receive the services
  • The cost of your services
  • Details about the management and coordination of your services.

If your Government Funded Package does not cover all your needs we can help you access private services or apply for a higher level Home Care Package.

Understanding Home Care Fees

There are three different types of fees for Home Care Packages. Some fees are determined by an income and asset assessment from Services Australia.

Your Home Care Package is based on your needs and your health. 

If eligible you may receive government funding to support the cost of your home care. 

You may also be required to make a financial contribution towards your Home Care Package by paying either: 

  • A Basic Care Fee or 
  • An Income Tested Fee 
  1. Income Tested Fee 

This fee is payable if your income exceeds a government determined threshold. 

  1. Government Funding 

This is the amount paid toward your package by the government and is determined by your assessed care needs.

We encourage you to research the services offered by providers and the costs associated with each service. Home Care Fees can be viewed online

Home Care can be delivered to you via a Government funded Home Care Package (HCP) or a Privately Funded package or a combination of both.

This is a subsidised service and eligibility depends on your individual circumstances. There are four different levels of Home Care packages, the level that is right for you will be determined by your My Aged Care Assessment. Find out if you’re eligible for the Government Funded Home Care Package. 

Classification 1 For people with basic care needs who require minimal assistance to remain living safely and independently at home. Suitable for light help with everyday tasks and occasional support.

Classification 2 For those needing a small but regular level of support, such as help with personal care, household tasks, or transport to appointments a few times a week.

Classification 3 For people with low-to-moderate care needs who benefit from consistent weekly support across a range of services including domestic assistance and social connection.

Classification 4 For those with moderate care needs requiring more frequent visits and a combination of personal care, allied health, and independence support.

Classification 5 For people with moderate-to-high needs who require regular, coordinated support across multiple service types, including some clinical or nursing care.

Classification 6 For those with higher and more complex care needs, requiring frequent visits from a range of care professionals and ongoing clinical support.

Classification 7 For people with high and complex needs who require intensive daily support, including significant nursing or allied health input to remain safely at home.

Classification 8 For those with the highest level of care needs, requiring comprehensive, daily, multi-disciplinary support equivalent to what might otherwise require residential aged care.

Pay for the services you need and work with your chosen provider to design the package that suits you. 

You have flexibility to increase or decrease services as your needs change. No government subsidy is paid for a private package, the cost of the service is paid by you direct to your provider.
